Cardiovascular disease

Common conditions such as heart disease, stroke and diabetes are all largely preventable. The leading causes of premature mortality from the above include obesity, tobacco, raised blood pressure, physical inactivity and raised cholesterol. 

Across Herefordshire and Worcestershire, there are areas of good practice such as enhanced targets for practices in the identification and management of patients with Atrial Fibrillation, hypertension and high cholesterol.

As part of our plans around cardiovascular disease we will continue to work with Public Health England, the local councils and Primary Care Networks to continue the focus at local population level. This will be achieved by:

  • Improving access to diagnostic ‘tools’
  • Working closely with PCNs, pharmacies, Public Health and voluntary sector to reduce health inequalities
  • Support lifestyle change and improve access to services
  • Continue to support and expand initiatives in primary care to identify people at risk and to effectively manage those people identified with a high risk conditions, such as raised blood pressure.
