Children’s Continuing Care

Children and young people aged 0-17, who have very complex health needs, either due to congenital conditions, long-term, life-limiting, or life-threatening conditions, disability or the after-effects of serious illness or injury.  These needs maybe so complex, that they cannot be met by through existing universal or specialist services which are routinely available from GP practices, hospitals or in the community.  For some children, a package of additional health support maybe needed, this additional package is known as Continuing Care.  The CCG is responsible for leading the process of identifying if a child or young person is eligible for continuing care assessment.

    If you think your child may need or require NHS Continuing Care support, please contact a community children's nurse, social worker or any other health and social care professional involved in your child’s care for advice. If appropriate, they will make a referral to the NHS Herefordshire and Worcestershire Clinical Commissioning Group (CCG) Continuing Care Service.

    Referrals are screened by a healthcare professional who is experienced in working with children and young people with complex health needs. If appropriate a comprehensive assessment will be undertaken involving parents/carers and all relevant professionals involved with the child.

    If your child is assessed as eligible for continuing care Herefordshire and Worcestershire CCG will ensure that the required clinical care is commissioned. Regular reviews are carried out to ensure that the commissioned clinical care remains appropriate.

    If your Child has palliative or end of life care needs the child’s consultant or palliative care team can refer through the fast track process to ensure that the care is commissioned as soon as possible.
